The clinic will now be open during lunch from 12:00 p.m. to 1:00 p.m.

• The Plymouth MASE Clinic will continue to be open and working their regular scheduled hours during the shutdown of schools.

• The Plymouth MASE Clinic will no longer close during lunch hours. They will be open all day.

Proactive MD advises if a patient suspects they may have come in contact with the Coronavirus and/or are experiencing symptoms, to please call the MASE Clinic and they can help direct them on where to go for care. The clinic does not have testing supplies for the Coronavirus at this time, however, they can assist patients on where to go for testing. Please call the clinic first and refrain from going in if you suspect you may have been exposed.

Medication Refills at MASE Plymouth

• MASE will no longer auto fill any medications.

• Please call MASE (574) 935-0127 or 1-866-258-6675 opt 2 with refill request.

• Allow at least 24 hours for your request to be processed.

• Make sure you know the name of medication you're requesting along with dosage and how you are taking the medication.
